The purpose of this form is acknowledgement of ICD's 2020 2021 tution policies.  Questions regarding this form should be directed to Mrs. Oetjen in the school office, stephanie.oetjen@icdschool.org . This Payment Acknowledgement must be completed prior to registration for the 2020-2021 School Year. Please complete all fields below. Required fields are marked with an asterisk *

ICD School Registration fees for the 2020-2021 school year are $250.00 per student, not to exceed $750.00. After submitting this form, you will be taken to a page containing a link to pay your 2020-2021 ICD School registration fee via credit card, debit card, or online check through My School Bucks.

I have read the school policy regarding tuition payment and agree to abide by the policy. I understand that ICD families are required to pay tuition through automatic withdrawal via a FACTS Tuition Agreement.

I agree to set up my tuition payments on the FACTS website, or I agree to have my existing FACTS account rolled over to the new school term. I understand that I will select online the option I have indicated above.

I understand that the returned payment fee charged to me by FACTS will be $30.  If my bank information or any other enrollment information changes at any time, I will contact the school office or FACTS Customer Service to provide them with the necessary changes.
